Role Description
This is a full-time on-site role as a Case Manager at Open Arms Community Health & Service Center in Los Angeles, CA. The Case Manager will be responsible for providing comprehensive case management services to individuals and families, including assessments, referrals, advocacy, and coordination of services. They will collaborate with a multidisciplinary team to develop and implement individualized care plans, and ensure the delivery of high-quality and culturally sensitive services.
Qualifications
- Previous experience in case management or social services
- Strong knowledge of community resources and services
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced and diverse environment
- Strong organizational and time management skills
- Proficiency in documentation and case management software
- Passion for advocating for individuals
